Licensed Practical Nurse (LPN) – Primary Care and Chronic Care Support (Ryan White HIV/AIDS Program)

Overview: Family Centers is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Primary Care and Ryan White team. In addition to providing routine nursing care, this role has a focus on high-risk and chronic care management support, referral specialist support, and improving compliance with treatment plans. Family Centers Health Care, a FQHC (Federally Qualified Health Center) provides primary healthcare, dental, and mental health services to children and adults living in Greenwich and Stamford with an emphasis on the special population living in or near public housing.

Summary Of Responsibilities
Primary Care Duties:
- Provide direct patient care in accordance with the scope of practice, including taking vitals, administering medications, wound care, and assisting with minor procedures.
- Collaborate with providers to ensure timely and accurate patient care.
- Document patient encounters, maintain up-to-date patient charts, and assist with patient intake and follow-up as needed.
- Identify and manage patients at high risk for complications, including those with chronic conditions such as HIV, diabetes, hypertension, heart disease, and COPD.
- Collaborate with providers, care managers, and other clinical staff to develop individualized care plans that address patients’ medical and social needs.
- Conduct regular follow-up and outreach to ensure patients adhere to their care plans and attend necessary appointments.
- Conduct assessments, provide education, and administer care offsite, including medication and wound management. Collaborate with providers to report findings and adjust care plans as needed based on home observations.
- Provide patient education on chronic disease management, medication adherence, lifestyle changes, and health promotion.
- Assist in improving patient compliance with treatment plans by identifying barriers and offering solutions (e.g., transportation, insurance, health literacy).
- Partner with care teams to develop and implement strategies to improve patient engagement and treatment adherence.
- Serve as a liaison between patients, providers, and specialists to facilitate seamless transitions of care.
- Work closely with providers to ensure timely and accurate referrals to specialists and external services.
- Participate in quality improvement initiatives aimed at enhancing patient care and clinic performance, especially in the areas of chronic disease management and high-risk patient care.
- Work within the Ryan White Program to ensure clients achieve health quality outcomes and connect with essential health resources for comprehensive, patient-centered care.
- Current CT LPN licensure
- Knowledge of population health, care coordination, and referral processes.
- Demonstrated proficiency with electronic health records (EHR), preferably Epic
- Bil ingual – Haitian Creole preferred
- Preferred - minimum of 2 years of experience in a primary care or outpatient setting .
- Preferred - experience in high-risk or chronic care management .
- Excellent communication and interpersonal skills.
- Strong clinical skills in primary care and chronic disease management.
- Ability to work collaboratively within a multidisciplinary team.
- High attention to detail and ability to manage multiple responsibilities.
- Strong organizational and time management skills.
- Compassionate, patient-centered approach to care.
- Must be able to stand and walk for up to 8 hours a day
- Must be able to stoop, kneel, crouch, extend without restrictions
- Must be able to lift a minimum of 50 lbs.
- Must have excellent hand/eye coordination
