Job Title: Case Manager

Location: Stamford, Connecticut

Employment Type: Full-time, On-site (Monday – Friday)

Pay Range: $100,000 – $120,000 (Based on experience)

Employment Status: Direct Hire

Position Overview:

A leading health system in Connecticut is seeking a dedicated Registered Nurse (RN) Case Manager to join their team in Stamford. In this high-impact, patient-centered role, you will work across departments to provide comprehensive case management for a high-risk adult population with frequent hospital readmission's. The primary goal is to reduce readmission rates by ensuring effective post-discharge care and coordination.

Key Responsibilities:

  • Assess, plan, implement, and evaluate individualized care plans in collaboration with multidisciplinary teams
  • Coordinate with social workers, physicians, and other health professionals to develop comprehensive discharge and follow-up care plans
  • Manage transitions of care, ensuring patients receive the necessary support and services to prevent re-hospitalization
  • Engage patients and families in care planning and education
  • Document all case management activities and maintain accurate patient records
  • Act as a liaison between patients, families, and providers to optimize care outcomes

Qualifications:

  • Active RN License in the State of Connecticut (CT)
  • Associate’s Degree in Nursing (ASN) required; must be enrolled in a BSN program
  • BSN Preferred
  • Minimum of 2 years of clinical experience in medical-surgical, ICU/acute care, or home health with adult populations
  • Strong understanding of case management principles and hospital discharge processes
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively across departments and disciplines

Schedule:

  • Full-time, Monday through Friday
  • On-site position – must be available to work 5 days a week in Stamford, CT
